Grants and Reimbursements Request Form
This form is for teachers and PTCO Volunteers to request reimbursement for PTCO purchases and to request field trip and individual teacher grants. Requests will be submitted to the PTCO officers for approval. Once your request is approved, we will mail your reimbursement to the address you provide.

You may only select one category for each payment. Please submit multiple requests if your expenses include multiple categories.
